accept the following forms of payment: MasterCard, Visa, personal
checks with a valid ID, travelers' checks, Perez Gift Certificates,

Gratuities

Our menu prices do not include gratuity. Should you wish to
leave gratuity for any of the Spa and Salon staff we ask that
you do so in cash. For your convenience, you may ask for a gratuity
envelope at the Reception Desk. |

while you are here.

that food not be brought into the spa treatment rooms.

Age
Requirements |

For
the Spa: Clients over the age of 12 are welcome to enjoy services
in our spa. A parent or adult guardian must accompany all
salon and spa clients under the age of 16.
For
the Salon: Children under the age of 16 must be accompanied
by a parent or adult guardian.
We
must receive permission from either a parent or guardian before
we begin a spa treatment or chemical hair service on any child
under the age of 16. |

Consultations |
| |
A consultation is a perfect opportunity to become acquainted
with the technician and address any questions you may have before
receiving your service. Our technicians are highly trained professionals
and will advise on the treatment best suited to your needs and
expectations. Certain procedures, such as lash extensions or
hair straightening, require a consultation prior to the service
and a fee may be applicable. Please inquire with Guest Services.
We ask that you complete a personal history and consent form
prior to receiving any facial or body treatment. This information
provides insight to your overall health and is considered confidential
and kept secure in your client record. Due to certain medical
conditions, your technician may advise against a particular
treatment or therapy and suggest an appropriate alternative. |

give the service you are accustomed to receiving at
Perez Salon & Day Spa, gift cards are a great gift!
We have beautiful gift cards available for your convenience.
You may choose a specific treatment, spa package, or
allow the recipient the freedom to select his or her
own treatment. Gift cards are not redeemable for cash.
Gift cards are not valid with any other discounts. There
are no refunds on gift cards, and we regret that lost
or stolen gift cards cannot be replaced or refunded. |
